The most common clinical presentations of venous insufficiency include spider veins and varicose veins. Spider veins are smaller, more superficial veins, resulting in a network of red and purple spider web like marks on the legs. Varicose veins are larger, swollen, blue and purple ropey veins easily visible on physical exam. Large varicose veins may throb; hurt or itch and on occasion may clot off or bleed through the skin. Varicose veins occur when the one way check valves in the superficial system of veins fail to close tightly. What is Phlebitis?

Phlebitis or deep venous thrombophlebitis means blood clots in the veins of the legs. This condition is serious for two reasons. Blood clots in the deep veins of the legs can break loose and got to the heart and lungs (a pulmonary embolus), which can be a life threatening problem. A secondary problem caused by thrombophlebitis is chronic venous insufficiency of the legs which can lead to chronic swelling, pain, and ulcerations of the legs. Thrombophlebitis usually causes pain and swelling of the legs and is diagnosed by ultrasound. Treatment is with anticoagulation (“blood thinners”), support hose and elevation. Surgery is rarely needed.

New technologies have produced a revolution in primary varicose vein treatments. Duplex ultrasound is now used for preoperative diagnosis, postoperative surveillance, and during many procedures. Ultrasound has also altered our understanding of the pathophysiology of chronic venous disease. Laser and radiofrequency saphenous ablations are common. Classic techniques, such as sclerotherapy, high ligation, stripping, and phlebectomy, have been improved. Magnetic resonance venography, computed tomographic venography, and intravascular ultrasound have improved diagnostic capabilities. New strategies like ambulatory selective varices ablation under local anesthesia (ASVAL) and conservative hemodynamic treatment for chronic venous insufficiency (CHIVA) raise important questions about how to manage these patients.
